Since upgrade to kde4.1.4 (and 4.2), whenever konqueror or dolphin are opened (either via commandline or krunner, or opening a folder from the Folder View plasmoid) they now start in fullscreen mode.  I can find no setting in either program to prevent this behaviour.

In 4.1.4, switching to normal (windowed) mode in konqueror and quitting meant that it would be windowed when re-opened (though it would return to opening in fullscreen after a reboot). I didn't check whether this was the case with dolphin. With 4.2.0, both filemanagers open fullscreen again after being switched to windowed mode and restarted.

However, issuing "kfmclient openURL <some local path>" on the commandline opens konqueror (which is set as my system default filemanager) in windowed mode.

This is under Gentoo x86 unstable; the issue has also been reported in Ubuntu Launchpad here:

Note that the Ubuntu bug was reported as happenin in Gnome.  From KDE on 4.1.4 I cannot reproduce this on Kubuntu.

Sorry, this is a red herring. I didn't realise it was a window-manager issue (which in my case is compiz + emerald, not kwin). I've followed the advice in the launchpad bug and resolved the issue. Konq still appeared with the menubar disabled, but I've changed that setting and it's survived a restart of the program, so I'm guessing it'll survive a reboot too.
