I haven't been able to really detect a pattern here, but quite often, especially when I type an url, krunner doesn't open the first action shown when I press enter and I have to click on the tile.

This is a known bug/behaviour, can't find a DUP for it now.

It happens, when you type exactly the same as suggested by autocompletion and the result is a desktop-file match.

I've also had it happen when typing urls and when typing the name of some little bash scripts I've created myself -- so I'm not sure about the deskop file part. The typing exactly the same string part is quite possible, I'm quite a fast typist.
