kstars, amarok, kate, are the most affected apps. In amarok there can be a delay of 5 seconds between a use event and the action. It only affects the GUI: music is playing fine, movies are playing fine, ....

This must be X or drivers problem, there is nothing in KDE that could affect this. I suggest trying MergedFB, TwinView (or whatever is the appropriate choice for the drivers) instead of the Xinerama implementation in X.
