I am using kde 4.1 from the split ebuilds provided by the gentoo kdesvn-portage overlay. I have noticed that when using the classic menu widget the menu entries do not update (installing/removing system applications and sometimes even after editing with kmenuedit, although sometimes it did update as I've noticed). This does not seem to be a problem with the kickoff menu which seems to update just fine (so yes, kbuildsycoca4 is ran after each update).

My quick workaround currently is after some update to just switch to kickoff and switch to classic and that updates it but I thought you should probably be aware of the issue. Hopefully it's not something gentoo specific :)
