I could not get Multihead 
(dualhead infact) support
working until I entered

KDE_MULTIHEAD=true
export KDE_MULTIHEAD

into my .xsession. This applies
to Xrandr 1.2

I got this from old bug report comment
of kwin, but I have the strong
feeling that it ought to be 
documented somewhere. Prominent 
if possible.

Multihead is not really supported in KDE. Please use a xinerama setup.

Have you tried using the xrandr command-line tool to configure a second monitor? One can typically issue 'xrandr -q' to find out what monitors are connected to your computer. After your figure out what the names of your connected monitors read xrandr's man page, paying special attention to the --output option and its friends. (A suggested companion for '--output' is '--auto' ;) )

xrandr and xinerame are both pretty spotty supported, and has random Xorg.con configuration requirement depending on the actual driver. Real multihead would be easier, and much prefered for the simple case of TV-out
