Version:           4.00.85 (using KDE 4.0.83)
Installed from:    Mandriva RPMs
OS:                Linux

If you place the location bar to the right of the main toolbar and lock the toolbars, every time you restart konqueror the location bar moves some pixels to the left, slowly taking over the main toolbar space and hiding the buttons.

I can confirm this on 4.0.98. If you try to put Main and Location bars on the same row, then Location bar will stretch off the right edge of the window, covering the Main toolbar and causing first half of URL to be clipped offscreen.

you don't have to lock the bars, just put them both on the same row, save the
profile, close all windows, restart konqueror and the the problem occurs.

Using KDE 4.1.00 (KDE 4.1.0) (KDEmod) in ArchLinux i686:

I can confirm this bug. (In order to properly close all Konqueror windows/instances to reproduce this bug, I had to run "killall konqueror"; otherwise you wouldn't notice the bug.)

Maybe it's related to other toolbar positioning bugs like bug 166984
(I experienced this bug in Kontact some time ago, the KOrganizer plugin toolbar was slowly moving to the left )
