I installed the packages from the mac.kde.org page and I chose the full installation. Most programs work, but all icons are missing. Kontact e.g. is still usable do to the text on the buttons, but it is convenient and looks horrible. Otherwise most things work beautifully. Good work :-)

Can you still reproduce this problem using a more recent version of KDE?

I think that in the recent versions this problem is fixed. In the recent version there is no kate and that's why i use older versions.

PIM packages are also missing, so I haven't tried it yet

this problem is fixed in the 4.2.0 packages from macports. Great work
