Version:           4.0.1 (using KDE 4.0.0)

When logging on to a KDE 4.0.1 session, two separate layout icons appear; right clicking one reveals 'KDE Keyboard Tool' with correct layouts displayed. Right clicking the other reveals 'KDE Keyboard Layout Switcher' with incorrect layouts displayed. However, clicking on the first causes the second to cycle through the same settings at the same time. There is no apparent operational problem; just a visual one.
This was not present in 4.0.0; has appeared in 4.0.1.

Is this issue still present in a more recent version of KDE, such as 4.0.4?

Yes, behaviour is the same as in 4.0.3; duplicate icon appears first or second 
and 'genuine' one near end. I say 'duplicate' because it does not always 
synchronise with the correct keyboard layout but usually cycles in sync. with 
the 'genuine' one.

4.1.2 (KDE 4.1.2) "release 44.2". When KDE4 starts I have only one layout switcher, and it is from KDE3. When I switch keyboard layout, then appears two keyboard layout switchers, the first is still from KDE3, the another is from KDE4. They are cycling in sync too.

Sorry, for me it isn't a bug. I'm checked out the Enable Keyboard Layouts in KDE3 - System Settings. Now I have just one Layout Switcher - works fine. :)

Same to me. I've got only one layout switcher icon.
