Have you got a filter which has "Send Fake MDN" as filter action? Also try to delete (after backup) all of your filters and see if the problem persists. No, "Send Fake MDN" is never occurred, but it turns out that the cause is the rule of type 'redirect'. If that filter applies, an MDN is sent.
