See attached screenshot.

This was confirmed in IRC. Using another style like this:

   kalzium --style="plastique"

Makes it work (obviously with the other style)

Confirmed for me as well, Motif style displays the icon normally!
Oxygen-related bug.

SVN commit 772648 by boemann:

Apply patch by David Benjamin fixing LTR layout of QToolBox
he gave me two options and i chose the one that mirrored the sloping 
divider.
It also fixes icons from appearing twice in QToolBox
