Hi!
the excluded servers are stored in 
$KDEHOME/share/apps/khtml/formcompletions

This file MUST be edited manualy, to revert/delete such a server.

IMHO all these files shouldn't be edited by users, because of possilbe damage. 

This should go somewhere in konqueror settings and / or contextsenisitive RMB on a html page of the excluded server.
