I spent much time cleaning up toolbars all over KDE, removing lots of icons. After upgrading from KDE 3.3.2 to KDE 3.4.1 lots of removed icons had re-appeared.

The strange thing is that the toolbar editor showed the original toolbar state, exactly as it was before the upgrade. The actual icons present on the toolbars did not match the toolbar icons that the editor was showing in the list of present icons. When I used the editor to add one icon, remove it again, and press 'apply', the toolbar would be restored to the state displayed by the toolbar editor.

I used this trick on all toolbars of all KDE applications, and managed to get them back in their pre-upgrade state.

Nor there can be with package managers/make install simply overwriting the old .rc files -- there is not enough information
