Version:           unknown (using KDE 3.4.0-1.2.3.kde, Fedora Core release 3 (Heidelberg))
Compiler:          gcc version 3.4.2 20041017 (Red Hat 3.4.2-6.fc3)

Using the Help/Configure Toolbars... menu item in any KDE application it is not possible to add any actions from the action list to any toolbar. 

Trying to do so just deletes the action from the action list and it never appears in the toolbar.
