Configure Toolbar dialog

When a new toolbar button is moved to the right list, it does not appear in the right list, while disappearing from the left list.  Clicking the apply button did not resolve the missing item, although it appeared in the toolbar.  Upon closing and opening the dialog, the item appeared in the right-hand list.
