This can happen without the user explictly opening the mail if the preview pane is in use and there is only one message in the outbox, or if a queue of mail is being sent when the message with the MDN request reaches the top of the list. If the option is set to "Always send" or "Deny", then the MDN is sent without user confirmation. The same happens if a message with a MDN request is saved in the drafts folder, and later reopened.
