Konqueror does not honour Ctrl Left-Click for selecting files if mouse is not
over the 'Name' column. It behaves as if the Ctrl key was not depressed. This
problem occurs in list type views eg 'Tree View', Info List View', Detailed
List View'. Shift Left-Click always seems to be honoured regardless of the
column being clicked on.

in svn trunk r818883 (~4.1 beta) you need to click twice to select a file

The behaviour in KDE 3 will not be changed: it is unmantained.

The current behaviour in KDE 4 has been discussed yet and I am sure it will not be modified.
