how to reproduce : 

mkdir /tmp/bug 
cd /tmp/bug
for i in 0 1 2 3 4 5 6 7 8 9; do touch $i.asc; done

open the KDE run command dialog
type /tmp/bug

Close and repeat if the icons appears properly

approximately one time over two, konqueror will not displayed icons for the asc files...

I can reproduce in KDE 3.5.6 - no matter what I do, icons are drawn only on hover.

kde 4.0.0 ubuntu gusty
konqueror-kde4 and dolphin-kde4 display icons, not the "lock" but at least they display icons :)

Still present in 3.5.9. I believe it may also be present in trunk r814642 (both konq and dolphin) because since the icons can't be found, they are replaced by the "no-icon" icon.

In Kubuntu 10.10 kde 4.62 and master/branch 4.6 compiled from sources this bug is fixed, an icon for empty document filetype application/x-zerosize is displayed.
