I still ignore in what circunstances it happens, i'll try investigate more in depth.

But Kate notes when a file has changed on disk, and when you try to save such a file, or focuses it, kate can warn you about it. We have yet no automatic reaction.
Unfortunately, without further info, there's nothing that can be done about this. If you have been able to figure out a pattern, when this happens, please add more detail. Else this report will likely be closed, soon.
When I implemented the feature, we discussed how agressive it should be, and ended up with what we have: * when a external file change is seen, the icon in the file list and status bar is changed to notify the user. * When the user tries to close kate or save a file, he is offered ways to deal with the situation. A possible improvement could be to offer immediate action as a user setting (show the dialog immediately when change is seen, for the current or all files. I can't remember if we suggest actions when a changed file becomes active, if not that would be a possible improvement too.
Kate in kde3 shows the dialog when the user changes to the file that changed and if the file is already visible it also displays the dialog to take some action. IMHO that makes sense and should be working this way in KDE4 too (I didn't check if it does).

Well, I guess I should have been more specific when asking for more specifics. What I'm wondering is, whether this is - a duplicate of bug #139402 - a duplicate of bug #104523 - something different Unless you (the reporter) tell us it's "something different", I'd propose to close this report.
