OS:          Linux (i686) release 2.4.19-4GB

Flag and mine icons don't appear when I click on a square with the left mouse button. It takes two clicks of the right mouse button before a question mark icon appears.

I'm running SuSE 8.1 (2.4.19 kernel) with Qt version 3.1.2-58. The Kmines version is 2.1.7e dated 5/11/2003. Both it and Qt were installed from rpms available on SuSE's site. Everything worked fine using an older version of Qt (can't remember the version number, but I think it was 3.1.2-6). I am aware that version 3.2 of Qt is available, but SuSE has yet to release updated rpms.

Same problem here after updating KDE 3.1.1 (SuSE 8.2) to KDE 3.1.3 (SuSE RPMS).  
KMines is now v2.1.7e.

The same bug seems to affect KPoker, where the cards are only displayed as 
silvery buttons. Noticed since update to 3.1.3 for SuSE 8.1, binary package. 
Rebuilding kdegames from ftp.kde.org package didn't help either.

The bug seems to be resolved in 3.1.4

seems to be Suse specific .... and fixed by now (?)
