Version:           2.1.1 (using 4.1.85 (KDE 4.1.85 (KDE 4.2 Beta2)), 4.1.85-1.fc10 Fedora)
Compiler:          gcc
OS:                Linux (x86_64) release 2.6.27.7-134.fc10.x86_64

In kalarm main window (not systray), when I disable an alarm, I see no visual indication it's been disabled.

The most likely cause is that the disabled alarm text colour is set to black (or similar). Go to Settings -> Configure KAlarm -> View and check what colour is shown for disabled alarms. The default as supplied by KDE is light grey, but perhaps either you or your distro have changed this. Please let me know whether the colour in the Configuration dialogue corresponds to what you see. If not, please provide a copy of your KAlarm configuration file ~/.kde/share/config/kalarmrc (or ~/.kde4/... perhaps).

Problem was that line was selected, so I couldn't see that it was grey.  When I selected a different line, then I could see it.  

When you select a line and a dialog pops up letting you disable the item (or make any other edits), if you select disable, there is no immediate feedback, because you can't see that the line is now greyed out, because it's still selected.

Fixed for KDE 4.1.4 and KDE 4.2 (SVN commits 897759, 897760).
