Many digital cameras have orientation sensors and put information about the image's orientation into the exif-fields.

Better image viewers show the image in the right orientation, only some older image viewers do not support this feature.

Gwenview has a bug in showing these Images: It first loads the image in the wrong orientation and displays it and then it displays a rotated version which makes Gwenview unusable for digital images (at least for me).

Just got some Canon 5D images (~12MPixel) from a friend:
With my 2GHz Athlon Gwenview needs more than two seconds to display the image in the wrong rotation, than it needs 2Seconds to show the correctly orientated version. 

Other programs (digikam, Kuickshow,...) work just fine.
